As a thank you for your continued support of our products, we are introducing our unique customer loyalty program. From now on, each time you order scripts, you will accrue reward points that can be redeemed for any of the items featured here.

How It Works
Each time you order scripts, your account will be automatically registered with the number of reward points for your purchase. You will receive one point for virtually every dollar you spend ordering our forge-resistant scripts.* You may order any item from our reward collection at any time using your earned reward points, or a combination of reward points and payment. You may also order any item to add to your collection by purchasing the item outright.

All items are available in Cognac or Black leather.

     Client Loyalty Program Terms and Conditions

1. The ScriptShield and National Rx Security client loyalty program begins on May 1, 2007.
2. Points will be automatically registered for virtually every dollar you spend on HologramRx prescription pads from ScriptShield or forge-resistant pads from National Rx Security. Points awarded are for prescription pad purchase price and do not include shipping and handling nor sales tax.
3. To redeem points and order items from the client loyalty program, call 1-800-510-1050 Monday to Friday, between the hours of 9am and 5pm EST.
4. We reserve the right to change reward items and point levels at anytime and without notice. New items or point levels will be posted on our web sites at hologramrx.com and nationalrx.net.
5. All reward orders are subject to availability. If your item is not available, you will be given a choice of another award of equal value. The item may be the most current model for that particular item at time of ordering.
6. Normal delivery time will be 4 to 6 weeks, but we cannot be held responsible if an item is backordered with a longer delivery period.
7. All reward items will be shipped to the shipping address we have on file for the client.
8. If one name appears on individual script orders, the reward points are registered and accumulated for that name and shipping address displayed on the script. Only the person whose name appears on the script may order reward items.
9. If two or more names appear on individual script orders, the reward points are registered and accumulated for that particular shipping address and anyone whose name appears on the script may order reward items. It is the client’s responsibility to assure which of the persons printed on the script are permitted to claim reward items.
10. If a clinic name appears on individual script orders, with individual persons’ names printed on the scripts as well, the award points are registered and accumulated for that particular shipping address and anyone whose name appears on the script may order reward items. It is the client’s responsibility to assure which of the persons printed on the script are permitted to claim reward items.
11. If a clinic name appears on individual script orders, without any names on the scripts apart from the clinic name, the reward points will be registered and accumulated for that clinic name at that particular shipping address. The person who has ordered the scripts for the clinic may order reward items. It is the client’s responsibility to ensure that the person ordering scripts is permitted to claim reward items.
12. Anyone redeeming points and ordering an item from the client loyalty program will initiate the request by calling 1-800-510-1050. The person ordering a loyalty program item will be required to complete and sign a redemption form which will be faxed to the client. This form will detail the name and address and signature to whom the item will be shipped. Reward items will be shipped to the shipping address we have on file for the client. ScriptShield and National Rx Security cannot assume any liability for any discrepancy in whom the client allows to order reward items on his/her behalf.
